The assessment and appendix are being provided separately due to some problems in compiling the various sources of information into the pdfs. Both documents are in Adobe Acrobat format. If you need a copy of Adobe Acrobat, you can find it here: Adobe Acrobat.
We also had some problems with embedding one font in the Acrobat file. If the headings display oddly, you can find that font here. If you install that font on your machine, the file should display properly. In most versions of Windows, copying this file to the [Windows]\fonts directory (where [Windows] represents the directory that contains Windows) will suffice to install the font. We were not able to determine which software package provided us with this font, so we suggest that you use this only for the purpose of displaying our document. Internet searches indicated, but did not prove that this is a freely distributed font.
